They're pretty much the same.  The only difference is that the backing for this item is flower-shaped so it's easier to screw or unscrew.  This is the 2nd generation model for threaded flat back stud.

Q : Re threaded diamond stud earrings: does this stud come with a 4mm diamond? What is the measurement in inches across the top side of the diamond? Thank you

4mm (1/4 carats) will be available in 2 weeks.  Please see the product photos and descriptions for the exact measurement.

Q : can I bring this to the store to have them piece my helix? What gauge would be best for the first time piercing?

We would recommend that you visit the piercing shop that you intend to get the piercing from and ask them if outside jewelry can be used.  Some piercers will only pierce with their jewelry.  If they allow it, then you can ask them what gauge and length would be best to purchase based on your specific needs.

A :

Push the post through your piercing first.  Once it comes out then you can screw on the diamond top onto the post.
